Los Angeles County Medi-Cal Transportation Services

As a Medi-Cal beneficiary in Los Angeles County, you are able to utilize transportation services to see your health care provider and to obtain medically necessary covered services at no cost.

Your Managed Health Care provider will work with you and your health care provider to find the transportation service that best fits your needs and to schedule a ride.

You can use this service to:

• Your doctor.
• Your dentist.
• A counselor.
• The pharmacy, to pick up medicine.
• Pick up durable medical equipment, such as a wheelchair or walker.
• Ongoing care, such as dialysis.
• Your home from the hospital.

Here are the steps to follow to schedule a Medi-Cal transportation in Los Angeles County.

Step 1 – Determine if You are Eligible

There are two types of Medi-Cal transportation: Nonmedical Transportation (NMT) and Nonemergency Medical Transportation(NEMT).

Non-emergency medical transportation (NEMT)

NEMT requires that a Physician Certification Statement (PCS) form be completed and signed by your Provider.

This transportation type can include transport by ambulance, litter van, wheelchair van medical transportation services, and/or coordinating with para transit.

Who can get Nonemergency Medical Transportation?

The forms of transportation are authorized when:

• Your medical and/or physical condition does not allow you to travel by bus, passenger car, taxicab, or another form of public or private transportation, and
• Transportation is required for the purpose of obtaining needed medical care.

Non-Medical Transportation (NMT)

NMT does not require a Physician Certification Statement (PCS) form to be completed.

This benefit allows for transportation to approved medical services by passenger car, taxi, or other forms of public/private transportation.

With NMT:

• You will have access to an unlimited number of round-trips per year.
• This benefit does not limit your non-emergency medical transportation benefit.

Who can get Nonmedical Transportation?

The forms of transportation are authorized for:

• Medically necessary covered services.
• Members picking up drug prescriptions that cannot be mailed directly to the member.
• Members picking up medical supplies, prosthetics, orthotics, and other equipment.

Step 2 – Schedule a Los Angeles Medi-Cal Transportation Ride

Medi-Cal transportation is scheduled through your Medi-Cal managed care provider (MCP).

In Los Angeles County, there are two MCPs – Los Angeles: LA Care and Health Net.

Schedule Medi-Cal Transportation with L.A. Care

If you have LA Care as your Managed Care provider, use the following information to schedule your Medi-Cal transportation ride.

To schedule NEMT or NMT services, please call L.A. Care Member Services at 1-888-839-9909 (TTY 711) and select the ‘transportation’ prompt.

You will be able to learn more about your transportation options and how to schedule a ride.

Schedule Medi-Cal Transportation with Health Net

If you have Health Net as your Managed Care provider, use the following information to schedule your Medi-Cal transportation ride.

Call ModivCare, formally LogistiCare at 1-855-253-6863.

Hearing-impaired members, call TTY: 1-866-288-3133.

You can call between 7 a.m. and 7 p.m. Pacific time, Monday through Friday.

Here’s information you should know before you call:

• If you need interpreter services during the transport, call the number on the back of your Member ID card for assistance.
• Please do not call more than 30 days before your health care visit to reserve a ride.
• If you are not able to call, a family member, caregiver, or doctor can call for you.

Additionally, please have this information ready when you reserve a ride:

• health plan member ID number.
• name and address of the medical doctor.
• appointment date and time.
• pick-up time and address.

How the Benefit Works

The types of rides you can schedule include:

Non-Emergency Medical Transportation (NEMT)

• Vehicles with wheelchairs and gurneys.
• Call 48 hours in advance.

Non-Medical Transportation (NMT)

• Car, van, taxi, rideshare, and mass transit.
• Call 24 hours in advance.
• Rideshare arrives within one hour.

You can schedule rides to any place that offers medical care or health care services. There is no mileage limit.

Step 3 – Wait for Your Ride

The final step is to wait for your scheduled ride. If they are any issues, call the transportation provider.

If they are not able to help you or they do not show up, call the managed care provider number you used to schedule the ride.

When you call your Managed Health Care Provider to schedule a Medi-Cal Transportation ride, you will be scheduled with one of the Medi-Cal transportation providers below.

Note: You cannot call the providers below directly to schedule a ride. You have to go through the Managed Care Provider Phone Number provided above.
